Referrals to RHCI

Every patient has the right to be referred to his or her facility of choice. A referral can be requested by hospital discharge planners, physicians, home health agencies and other healthcare professionals. Individuals may also refer themselves or be referred by a family member.

Most people are admitted to RHCI from an acute care hospital. If you wish to be considered for admission to RHCI, notify the discharge planner from the acute care hospital, who will contact us. A referral nurse from RHCI will visit you to discuss your rehabilitation needs and determine if you meet the admissions criteria for a rehabilitation hospital. If you choose to come to RHCI, we will work with the discharge planner, your insurance carrier and/or your physician to make the necessary arrangements for admission to RHCI.

In some cases, individuals can be admitted to RHCI without having to stay in the acute care hospital. Someone who is living at home, but losing the ability to perform daily activities, may be a candidate for admission to RHCI through the Direct Admission Program (DAP). These individuals must meet specific criteria established by Medicare to be accepted into the program.
