Cardiac Rehabilitation

RHCI provides a full continuum of cardiac rehabilitation services to help those with heart (cardiovascular) disease return to active, productive lives.

Phase I (Inpatient)

RHCI’s Phase I Program provides a comprehensive array of services to maximize functional independence and improve quality of life after open heart surgery, a heart attack (myocardial infarction) or other cardiac event.

  • Individualized, outcome-oriented treatment plan
  • Coordinated, intensive therapies to promote maximum functional improvement
  • Comprehensive educational program for patients and their families
  • Supportive resources that assist with the emotional adjustment to heart disease and promote associated lifestyle changes.
  • Cardiac rehabilitation stages that provide clear, graduated guidelines for daily activity and exercise
  • Nutrition education and counseling
  • Therapeutic recreation services
  • Specialty services, ordered by the patient’s physician as needed, including cardiology consultation, cardiac telemetry, etc.
  • Extensive case management with links for ongoing treatment and support after discharge

Phase II (Outpatient)

  • A 12-week Healthy Hearts Outpatient Cardiac Rehabilitation Program
  • Group cardiovascular conditioning, education and support
  • Maximizes recovery and reduces the risk of further cardiac events through positive lifestyle change
  • Continuity of care with knowledgeable RHCI clinicians

Phase III (Outpatient Maintenance)

  • An ongoing Outpatient Cardiac Maintenance Program
  • Exercise and support program
  • Offered as a self-pay option for those wishing ongoing support as they promote cardiovascular health
